OVERVIEW

Population Health: What's It All About?

Disease management has long been considered a solution to help manage chronic conditions. However, as today's healthcare environment becomes more complex - large numbers of uninsured, aging of the US population, chronic illnesses appearing earlier in the lifespan - it is becoming increasingly clear that in order to come up with appropriate solutions to the healthcare crisis in America, we must examine the situation from a more comprehensive perspective...that of population health.

Population health provides a broader focus that includes the impact of environmental, social, cultural, and economic factors on overall health care and quality of life. It provides a context for developing, implementing, and evaluating appropriate changes to the system and policy that will have a positive effect on health care quality, access and outcomes at the population level.
