AUDIOCONFERENCE REGISTRATION INFORMATION
You may register separately for the training audioconference (2:00 pm to 3:00 pm eastern). If you register for training audioconference only, you will also receive the certification examination which you may take as a practice exam (which you self-score during the Test Review section of the audioconference). You may also participate in the review audioconference (3:15 pm to 3:45 pm).
You may register separately for the certification examination (2:15 pm to 3:15 pm eastern). If you register for the certification examination only, you may also participate in the review audioconference (3:15 pm to 3:45 pm).
Alternatively you can register for the training audioconference and certification examination package at the rate of $575.
Individual two-year certification is normally $500. Accordingly, your certification via this class is a good value. If you don't pass the test, you may take a make-up test at any subsequent time for $200, or you may send in a report which you found on your own and critique it for $200.

NO LIMIT TO THE NUMBER OF LISTENERS AT EACH REGISTERED SITE FOR AUDIOCONFERENCE
There is no limit to the number of individuals who can gather to listen to the audioconference in a single location for each registered site. However, only one site can be offered per registration. It is impermissible for an organization to purchase one registration and to operate multiple sites through an internal telephone system.
INDIVIDUAL REGISTRATION REQUIRED FOR CERTIFICATION EXAMINATION
Every individual taking the certification examination must register separately.
METHOD OF PAYMENT FOR TUITION
Make payment by check (to Health Care Conference Administrators, LLC), MasterCard, Visa or American Express. A $20 fee will be charged on any returned checks.
TAX DEDUCTIBILITY
Expenses of training include; tuition, travel, lodging and meals, incurred to maintain or improve skills in your profession may be tax deductible. Consult your tax advisor.

CANCELLATIONS
